Looking at the document "Apple URL Scheme Reference," the "Phone Links" chapter, I see a reference to RFC 2806, which defines the tel scheme. I skimmed the RFC _very_ briefly, but it looks as though your first step is to stop stripping the +. Stripping the spaces seems to be required (or substitute one of "." "-" "(" or ")").
