I found the issue, it lies in with using SteerMouse (current version) Ver 4.0.3 (Jan 7, 2011). Somehow this mouse configuration app causes the mouse over & mouse out to stop functioning globally in Mac OS X 10.6.6 (current version) when switching a user, or when calling the Login Window.
Just a note for the archives.
