I wouldn't even go as far as saying you 'have' to have scaled images, since
Icon Composer (find it in /Developer/Applications/Utilities/Icon
Composer.app or in /Applications/Xcode.app/Contents/Applications/Icon
Composer.app for App Store Dev tools installs) can do this for you.
Of course, if you want the icon to look different at different sizes then
you'll need to make different ones.
Personally, I've always tried to make my own icons, but generally I always
find icon designers do a better job, and always quicker as well.
Someone who I know well, and has done numerous icons for apps and websites
is Dan Deming-Henes ( http://strandeddesign.com/ ). He's done apps like
Quicksilver, Droplr, Transmission. Always quick to respond, and quick to
get new icons out.
I can't comment on price since... let's just say he likes supporting open
source projects :)
