My solutions has custom code for the Icon size handling on control size change, since this didn't work so well with code-generated views and PDF-Based Icons templates.
I just did a test creating a toolbar with IB and it works exactly as you wish it to. Be aware you need to add "Rounded Textured Button" for a PushButton to have the correct apearance. If you drag in a NSSegmentedControl it automatically gets changed to the correct style, as do Popup Buttons. I'm using the 10.8 SDKs.
