I just saw this message. I'm no kind of expert at Cocoa/IB, but I'll have a stab at this since you didn't appear to get any responses.
Generally, yes, the actual interactive item is the one VO needs to know about. In your example, the button is what VO should focus on. Is this a custom UI control? I ask because Cocoa's default UI controls are accessible out of the box, and only need special attention if you you do something like set an image for the button to display and don't offer any accessibility text for VO to speak. If you're still having trouble with this, email@hidden is a great resource for this kind of question.
