I don’t think you’re missing anything, but “broken” isn’t quite the right word. In a non-adversarial situation, you can come to some accommodation with the other developer, depending on what kind of extension you want. (If it’s completely app-specific anyway, using a longer extension that suggests your app name might be a possibility. If it’s an extension that is already in use, there’s not a lot you can do about it.)
If it’s adversarial, you don’t get much leverage about the extension, though I suspect you might be able to do something about an app-store app that stole your UTI, if it was clearly malicious. Even in the old days, where you had to apply to Apple for a unique creatorID, I don’t recall there was any enforcement if someone used yours against you.
