What you're talking about is a Device Link Profile (straight CMYK-to-CMYK
conversion). ColorThink and a few other apps can create one. Be forewarned,
most RIPs will not accept device link profiles.

ColorBlind Matchbox Profiler and ColorBlind Professional will both make ICC Profile Links.
