Carl - I use a Creo ES Supreme, but have not produced a custom profile.
The out of the box raw scan using ES Ref/Trans default ICC profiles
produces a very good scan which is near linear workspace greybalanced
(very close to true neutral R=G=B right across the scale).
The 'lut ICC crt' input that you mention is the one needed for scanning
an ICC target. This provides an untoned scan which is flat in preview.
Check the enpoints table...it is probably off when LUT ICC CRT is
chosen.
The CreoScitex has a very good site, with Shockwave simulations of using
the scanner and for setting up an ICC workflow:
