Yes, I experienced this last week. I do not think it is a certain type of
monitor problem as it was with an Apple Cinema Display and not a Mitsubishi.
I have not had time (and probably won't in the near future either) to test
and find exactly the problem, but here is a bit of data if you're inclined
to investigate.
Previously, I had a G4 733 with 9.1. Last week, after changing to a dual
800 with 9.2.1 Colorsync 3.0.4. I saw the square you mention. There has
been no change of display or Eye-One, just CPU. After seeing the square, I
scrolled through the various old monitor profiles (which I sometimes keep
for comparison and testing) and the sqaure changed color with each profile.
There were some nice red squares a couple of greens, and blues. All the old
profiles reproduce the square, which seems to point to an OS/profile
software/Colorsync combo difficulty.
In order to correct the problem with new profiles, I simply dropped the
brightness a bit during the brightness adjustment stage.
