Cheers, Thomas - these are good answers and I thank you, even though I do
not like all the answers - at least I might be able to formulate some better
workflows, and perhaps others my be more informed on the subject.
So my sincere thanks to Thomas for taking the time to explain, and for Neil
for bringing up this issue.
Constructing graphics is not a simple thing, and there are many workflows
and many ways that art is constructed. As you mention, I have used the more
regular separated output for Duotone, and do not do a lot of composite
prints or conversions - but many do.
