Repro film routinely has d-max higher than 4, particularly film for engraving, which usually shoots for a d-max closer to 5. X-ray film can also exceed a d-max of 4. It's unlikely that any film exposed in a normal camera will ever get that dense, but densities in excess of 4 do exist.
