Actually, I don't think this is unique to ICC profiles. A font is only
included once (whether subset or not), even if it's used more than once
in the document. And if you place the exact same image more than once,
it increases the resulting PDF file size in a negligible way, rather
than actually being embedded in the PDF multiple times.
Example, 25MB RGB image saved out as PDF using High Quality setting,
yields a file 782,200 bytes. Placing the same image four more times,
total of five and exporting with the same setting, yields a file
782,501 bytes.
