There is no single format. Every manufacturer has their own proprietary method of storing the ones and zeros that come out of a sensor readout, and they frequently change their minds as to how they want to go about doing that in each successive camera generation. There are more raw formats than can or should be researched for a paper about workflows unless you just need several hundred pages worth of mind-numbing padding.
You might be able to find out which commercially sold camera was the first to offer raw as a user selectable option (no, I don't know), but the first "raw file" was more than likely written in a lab somewhere when the CCD was being developed.
