They'll always look OK when you proof them because the proofer because it's showing perfect printing conditions. But using a standard profile will mean that you images get hosed every time the printer loses control of gray balance - which is most of the time :-)

You'll find quite a few double page spreads in glossy magazines that
have one page sepia and the other looking like a cyan/black duotone.
They indicate the clients who think that having a profile and a copy
of Acrobat replaces the repro guy in the production chain...

You need to create (or find) a profile specifically for 4 colour black
and white images which is based on the same characterisation data but
produces a nice long and wide K and very little CMY - making it much
harder for the printers to hose.

In addition to Martin's suggestion above, another tactic I often use is to bias the separation slightly in a certain direction, e.g. a warm tone for portraits. The press will oscillate no matter what you do. I rather have it oscillate around a preferred color and not around 'neutral'.
