DeviceLink from CMYK data

I have 2 sets of CMYK numbers; the start values of the IT8.7/4 chart and another set after the chart has gone through a color transform. Is there a way to make a devicelink profile with just this information? I want to get the separations as represented in the second set of numbers, not CMYK values that have passed through LAB and thus get re- separated. Is this possible? All the tools I have that make device links will only work with 2 existing icc profiles, but I just have these CMYK values. Any ideas?
