I have a feeling this will be a long thread....
My opinion-
I like images to come in as RGB with an embeded profile. If there is no
profile embeded then start asigning profiles and toggle the preview on and
off until you find a profile that doesn't change the image. This is most
likely the working space for that image.
Then convert to your desired CMYK profile.
I will do this CMYK conversion even if the image is already CMYK.
Remember that you need an image to have an embeded profile in order to do an
accurate conversion. You wouldn't ask someone to bake your favorite brownie
without a recipe - so why would you ask someone to print a photo without a
profile.
