Well, there are a number of ways it's possible to get blue scum dots and they may not all be related to the same issue. It's really a complicated sequence of events to describe in email. For this v4 scum dot problem to get triggered requires a number of things to be true: three things need to be true in the Mac OS print pipeline and CMM, one thing needs to be true in Photoshop or Lightroom, one thing needs to be true in the v4 profile, and one thing needs to be true in the print driver. If anyone one of those were false, the problem wouldn't happen. So it's a "wow, bad luck" kindof problem.
