Various output, usually with my Printer Test File images. Mostly from my own output device (Epson's, 3880 or 4900).
I do have more than one calibration target for the NEC for differing papers, but the major differences is usually contrast ratio.

Usually not, I'm soft proofing however to achieve a match between it and the final print. All the profiles are custom built.
