I have found Tim Kientzle's "A Programmer's Guide to Sound" stands out as
a very straightforward and practical introduction to this field. He
gives examples which are fairly platform-agnostic along with concrete
(though outdated) code for playing sound and MIDI on Windows, Mac and
Unix.
