It is nearly impossible today to know what was the intent of
the MIDI specification... at least those parts written in the
first decade <g>.
What we do know is that according to the General MIDI and DLS
synthesizer specifications, volume and expression are to be
used exactly as Mr. Kent described.
However, the text of the MIDI 1.0 Detailed Specification says
"Expression is a form of volume accent above the programmed or
main volume" ... which would seem to mean that you can only use
expression to increase the volume, which is not very useful.
I doubt that was really what was intended, but who knows?
Anyway, the common practice today is what Mr. Kent describes.
