First, reboot and see if that clears the problem.
If that doesn't help, the next thing I would do is this:
sudo rm -rf /Library/Audio/MIDI\ Drivers/*
This will remove all the MIDI drivers, which seem like the most likely
source of a crashing MIDI server. You may also have some drivers
lurking in:
/System/Library/Extensions
But there is no simple command line for getting rid of those. You'll
have to eyeball the listing of that directory before deciding what to
delete if anything.
If this helps you bring up your app without crashing the MIDI server,
then the problem was most likely in one of the MIDI drivers. Add them
back one by one, running your app after each, to see which driver is
the problem.
From the crash log it looks like Apple's serial MIDI driver is the
culprit, but I would investigate the other drivers as described above
anyway.
