I fixed this. Checked the docs and the mailing list and found that I
had made what seems to be two common errors.
1. audiostreamdescription mNumberChannels semantics when non-interleaved
2. calculate output buffer size property of an audio converter
semantics is based around the size of the 'buffer' as in the
audiobufferlist
