Do you have QT7 or Tiger installed by any chance? I sent the
ComplexPlayThru app to a user who was having problems with my own app after
QT7 install (on 10.3.9). He claimed it simply produced a buzzing sound. I
wasn't sure if he was doing something wrong and managed to fix my own app,
so I didn't follow up with him.

My own problems were due to some change in the behavior of AudioConverter
for sample rate conversions. I think possibly the results returned when you
query the converter for the amount of output a given amount of input will
produce has changed with QT7. Switching from ConvertBuffer to
FillComplexBuffer solved my problems.
