The initialise "cludge" - yes that's entirely reasonable - AULab does
this. Because the AUHAL units are so dynamic, they tend to ignore the
init state. However, I also think there's a bug here in AUHAL that we
should fix - it would be great if you could write it -
bugreporter.apple.com
- describe what you did/wanted to do - it didn't work, so you had to
initialise the AU to get it to work.

once with only the output to cache the stream format, and a second
time with the whole kit and kaboodle.

AUHAL is a little weird in that most AUs you should only
initialise after you are ready to go... but because it has to deal
with changing device stuff, its generally best to just initialise
this guy and forget about it.... all of the format/device
properties can be set on this when it is initialised...
That's the only thing that comes to mind
