You could also build and install the AudioReflector driver. It has
completely configurable I/O and Stream formats (of course, you will need to
configure it with no input for your test). This may be easier for you if you
don't have a suitable USB device to test with.

Thanks for the suggestions.
The trouble was not having a device with output but not input so much as it was
only having an output device (ie. no input anywhere on the machine). What I did
in the end was deactivate my built-in audio drivers by removing the appropriate
kernel extensions and rebooting. Then I plugged in a USB mike (which is the
only one-way device I have) and reproduced the bug.
replacing the kernel extensions, with correct permissions, and rebooting got me
back. Having an output device would have been even better, but I think I
tracked down and fixed the problem. If the user reports that the problem is
persisting, I'll go buy some USB device with only an output and try again.
