How can I make a simple app that plays a few MIDI notes?

Take a look at the /Developer/Examples/CoreAudio/SimpleSDK/ PlaySoftMIDI example. It is very basic and gets you up to speed fairly quickly – once you get the basics it's fairly easy to do MIDI.
