Well, we can reproduce this - screeches like this are typically
because NaNs or infinities are getting into the device. We're not
sure yet of the cause of this. Filing bugs for these issues are very
helpful

Hi Peter,
It seams to me that I have a problem like yours but mine is
reproducible with Audio files and I still cannot to find concrete
steps to reproduce. Bellow is my previous request to Core Audio List.

Subject: Very Loud Noize
Hi all,
I have a serious problem but it is not well researched, probably
something is wrong in my code but I do not know where to start. In
short I have the next graph:
DefaultOutputUnit <-- AUMixer <--*a lot of input sources that are
added and removed when graph is running
I add the source to the mixer to start and remove it from to stop.
But sometimes (it is very hard to reproduce) I hear very-very loud
noise, it's volume does not depend on volume of the device and is
reproducible even when the device is muted.
sorry if the information is not full, I will be happy to any
suggestions

I will be very thankful to you if you send me your MIDI file, it
will significantly help me in researches and workarounds.
Thank you

Hi,
I just posted the following bug report into Radar - Bug ID 5042146
and am reposting here for your info:
Pitchbend in MIDI File Causes Massive Audio Overload on Intel
The enclosed MIDI file causes a massive overload of the audio
system on both Intel MacBook and Intel iMac at a certain point in
the file. At this point a pitchbend is sent hard to maximum and a
very loud screech can be heard through the audio system. This
screech is *much* louder than expected and is very serious. Do not
play this file loud!
The file can be played using the PlaySequence tool in the
CoreAudio SDK. The command is: ./PlaySequence -p -t 2210 Buggy
\ File.mid
In other words the loud screech can be heard just past 2210 beats
into the file (about 2215).
The overload lasts for many seconds. The file plays correctly on
an external device. It also plays correctly under Rosetta.
End of Bug Report
I can send the file to anyone who requests it but repeat the
warning. The screech appears to play at full volume regardless of
the actual hardware volume. I first heard it through headphones
and nearly had a heart attack. I was worried that my headphones
were damaged. We accidently played it through my sons iMac after
removing his attached stereo system and again the result was
extremely loud.
