The usual cause for this sort of error is that an IOProc hasn't be
added with AudioDeviceAddIOProc prior to being started with
AudioDeviceStart. I presume that you are using an instance of AUHAL to
handle the IO, so I'm somewhat puzzled how something like this might
have come up.
Perhaps you could explain a bit more about the problem?
