If I were coming at this problem i'd probably use a pair of midi
channels, one for the usual notes and one for the 'in-between' notes.
I can imagine running two synths, one detuned to handle the 'in-
between' notes, or supporting all 256 notes in a soft synth.
As for the physical side of things i'd be looking to bodge from a
pair of existing synth keyboards.

Actually, if you want quarter-tone, you probably don't want anything
like MIDI:-)
There's really nothing about MIDI that makes it unsuitable for
quarter-tone use, other than that since there are only 128 notes,
you'd only get a little more than 5 octaves.
MIDI note values are simple numerics: 0, 1, ..., 127. They are not
pitched, so you could use them to send quarter-tone data.
