A client application is calling MIDIPortDisconnectSource. It's not the MIDIServer, and not the CoreMIDI client framework.

Same things happens with /Developer/Examples/CoreAudio/MIDI/ SampleTools/Echo.cpp, btw. So I think it's the MIDIServer's fault rather than the application's.
