I'm not quite sure what you mean, changing the "configure speakers" still works for me (at least on 10.5.2). If applications fail to follow the settings (like iTunes), you're probably better off filing a bug against the app rather than CoreAudio. Applications should register to listen to the kAudioDevicePropertyPreferredChannelsForStereo property if they want to see the changes as they occur in AMS.
