I had the same problem as you, and I had sent a complain to Apple via the comment link in the bottom of the document, because I had been trapped in this documents for over 2 weeks!!!
I wish apple could create some real document than just put some comments together.. I hate this!
Should I do something else to let them understand me? and make the change for the documents?

I've come across it in some sample code, but in the I can only find mention of it in the Core Audio Overview. The 'SDK Documentation' as downloaded from http://developer.apple.com/audio/Core_Audio_SDK_Documentation.zip does not seem to mention it.
