Thanks for the response. We can't reproduce this issue with our devices. The below is the log from our customer, the first line printed the path of the file (and name), the next line is the error code.
18:25:30.671 Recordcallcont recording file=/var/mobile/Applications/F9A0D4CD-192F-46FD-B3BB-277689184659/Documents/record/mori2-hg@2010/04/26 18:25:25.caf 18:25:30.684 OngoingCall.m create audio failed!! err=70726D3F
To me, the path "/var/mobile/Applications/F9A0D4CD-192F-46FD-B3BB-277689184659/Documents/record/mori2-hg@2010/04/26 18:25:25.caf" looks good. Could this be an issue? Or could this path be read only in some case? Another customer does report after restore the device, this issue is gone.

it means you don't have permission to write a file at that location. If you don't think that is your problem, then can you
reproduce this, and send us a bug report. As far as I know, there aren't any issues here that we know of at least.

We call this with kAudioFileFlags_EraseFile, so I do not see any permission issue here. And the document also does not mention the permission issue. Anyone could help?
