Thanks to everyone who helped with suggestions for this issue.
It turned out that Qt was filtering kEventWindowActivated and
kEventWindowDeactivated events on non-Qt windows (like the PACE dialog).
There's a patch against Qt 4.6.3 here if anyone ever needs it:

Interestingly PACE doesn't use the kAudioUnitErr_Unauthorized result code
that Michael mentioned, it throws up its own modal dialog inside
OpenComponent().
