iOS Line Out (docked) volume

In general iOS seems to save/restore the volume settings for your
various routes. One exception seems to be the Line Out (docked) mode.
It simply takes whatever volume the Speaker is currently set to, and
*hides* the volume slider. (MPVolumeView). This seems like an
oversight, since generally you would always want your line-out volume
maxed.

This is resulting in poor behavior for some of our apps, though I
notice the system player seems to set the volume to 0dB explicitly
when it notices the LineOut situation.

Any ideas on what should be done here? Testing with iOS 5.1.1 and 6
