I've dug through the list archives another time, and although I found a lot of recommendations to use extaudiofileread for the convenience of automatic conversion, I haven't found anything that would indicate a problem reading stereo data.
In the mean time, I've tried using an interleaved client format using vDSP_ctoz to get left and right data into separate arrays, but the problem still remains - I'm getting duplicate data in both channels. Ie, it's not reading stereo. It appears to be reading just one channel of audio and using it twice in the output buffer list.
It seems like a lot of folks on the list both use and recommend ExtAudioFile – someone must have something to say about this?
P.S. I've messed around with the client ASBD endlessly, in every possible way; I'm not getting an error when I set it, and I am getting the right sound... just minus a channel. Other paths I've tried are saving the source audio in many different formats. Still no luck.
