listening in on default input and output devices?

Hello,

This is mostly out of curiosity: is it possible (on OS X and/or iOS) to listen in on what comes in and what goes out via the default/current input and output devices? I guess the answer is yes for OS X given that I've already used Apple Jack to record streaming shows.

The question is of course inspired by the fact that the AppStore only lists (paying) call recorder apps that seem to rely on VOIP or operator-provided means - although that could of course just mean that Apple won't allow 'sneakier' apps.
