ws_hash_line_clear and Kernel Panics

When you are hit by a kernel panic that states:

tws_hash_line_clear: out of sync

as defined in osfmk/vm/task_working_set.c.

Does it mean an issue with the Hard Drive for the VM (Disk Utility says the drive is fine), RAM (Apple Hardware Test says the RAM is fine) or code?
