It's not clear from the above, but I think you may be trying to interpose after the stack has already been set up - you cannot do this. Your kext must be present at the time the block device is being matched if you want to participate.
If you were successful matching against a partition from a disk image that you mounted after loading your kext, try installing your kext and rebooting.
