When you say the card is not detected, what do you mean?
Does the card not appear in the Keychain (even with the "show keychains"
button pressed)?
When you connect the card reader, a process named pcscd should start
running. When you insert the CAC into the reader, a process named
CAC.tokend should start running. You can watch for these using the Activity
Monitor utility (in /Applications/Utilities). In the Activity Monitor main
window, enter pcscd or CAC in the filter field, and choose "All Processes"
in the show popup. Also, monitor your system.log file for entries like
this:
Jun 23 11:40:42 Pauls-MacBook com.apple.SecurityServer: Token reader CCID
Smart Card Reader 0 0 removed from system
Jun 23 11:40:52 Pauls-MacBook com.apple.SecurityServer: Token reader CCID
Smart Card Reader 0 0 inserted into system.
