Carlos,
First, if AMSmartCard is running, then your CAC should appear in the
Keychain.
Remember, smart cards DO NOT automatically appear in the Keychain Access
window. You MUST make sure to click the "Show Keychains" button in the
lower left corner of the window to make the Keychains pane appear.
If you have installed ADmitMac for CAC, please contact Thursby's technical
support department, and they will help you:

If you are absolutely sure you don't see the smart card appear, AND you did
NOT install ADmitmac for CAC, I would recommend removing the Thursby tokend,
and trying with just the Apple CAC. To remove the Thursby tokend, REMOVE
your smart card, then open a terminal window, and enter this command:
sudo rm -rf /System/Library/Security/tokend/AMSmartCard.tokend
Now insert your CAC again.
