I look at it as something like cellular broadband wireless; sure I'd like to have it built into the machine, but I'd also like to be able to change providers, or change standards easily. My only gripe with the design of the new laptops is that the USB slots are pretty close together, so it's hard to have more than one wide USB device plugged in at a time.
I really doubt it's a purely aesthetic thing, you're right, the slot wouldn't look that bad; I think the decision likely had more to do with the number of people who would use it, and the trade-off in terms of internal space. What do you take out, re-arrange, or modify to fit it in.

Apple keeps saying they're big on Enterprise, but a smartcard reader seems like such an easy thing to do that would go a long way to proving it and helping so many people solve these problems.
