It all started with iOS 5. I have a Verizon iPhone 4 and tethering was so good that I dropped the data plan from my iPad. As soon as I updated to iOS 5 I have to be on the hotspot on setting page for the iPad to connect. It also looses connection but I can restore by turning hotspot off and back on. Sometimes wifi on and off on the iPad has to happen before off then on on iPhone.
Interestingly, it does not seem to disconnect as often when I'm using my MBP but I must still be on the hotspot on settings page to connect. When using USB I have no issues whatsoever.
