Have seen an issue with the GM and permissions causing what appears to be a failure according to Disk Utility. As it turns out, Disk Utility actually is erroneously reporting failure but the byte count on source and destination are the same.
I have installed Mt. Lion (lock stock and barrel, server too) on a USB thumb drive as well as created an installer on a thumb drive from the Mt. Lion download. Both worked nicely.
