There appear to be hardware related issues too in how Apple uses the two antennas versus other hardware makers.

Other hardware makers leverage the two antennas to carry two different channels. Apple uses them both to strengthen a single channel. Since voice/data simultaneously on CDMA requires two channels the iPhone can't do it.
