I have seen similar when logging into our Citrix system via Citrix access gateway a.k.a. web page menu and system.
From my experience this seems to be a problem with our you specifically. If you create a new fresh user profile, and try to login with your CAC (me, PIV), does it work? Mine did. Then back to my normal profile, broken still.
The solution is not simple either. Creating a fresh profile can work for a while but eventually breaks.
I have a user called "CAG" on my home Mac just for this reason. I delete the user when it fouls and create a new one to have it work again.
