Only Apple can answer that questions. Bug reports are your friend.

I've never gotten FSEvents working 100%, I still get reports from users saying
it's stops working mysteriously for some folders.  Have you ever seen this?

I've never had any problems with FSEvents, but I don't use it the way most other developers do so that's probably not much help. (I use it in my backup products; I only collect historical FSEvents from some relatively recent date in the past.)


Is it possible that something else is happening to the watched folder, like being replaced or moved? In the one occasion where FSEvents seemed to have stopped working it was actually because the alias to the folder was confused (the user had reformatted and restored their drive) so it was pointing to the wrong location.
