Is the root item of your package payload the application itself and
the default target location /Applications? And is the application's
icon not set through a bundle but through a Resource Fork?
If the answer is yes to both, then it's a known bug in PackageMaker,
splitforks and Installer.app (pick one of the three depending on the
OS version).
A workaround is to use / as the default location and have the
Applications/MyApp path be part of your payload.
