Unfortunately, it's not covered by the "documentation".

Basically, you add your files to the package and then you click the
iTunes store arrow on the right of the Patch: label for the file or
folder and you follow the instructions.

"Surprisingly", this does not work well with the version of
PackageMaker I'm playing with on Mac OS X 10.5.

Also, I doubt this is as powerful as the patch feature which is/was
available in InstallerVISE (where you can build a patch for multiple
previous versions of a file).
