It's listed in the caveats section of the sample's documentation.

Due to a bug in the system software, this sample may be unreliable when run on Mac OS 9.1. The problem is that MPRemoteCall sometimes deadlocks the system on Mac OS 9.1. The only workaround is to not use Mac OS 9.1. The problem does not occur on Mac OS 9.0.x and is fixed on Mac OS 9.2.x.

The problem is that 9.1 can mess up the RPC associated with MPRemoteCall, which causes the MP thread never to unblock. I might have been able to work around this in OTMP itself, but other MP services (such as MPAllocateAligned!) use MPRemoteCall, so there was little point.
